To be eligible for home health services under Medicare Part A or Medicare Part B, a patient must meet one of the following conditions:
- You are unable to leave your home except for short outings, such as doctor visits. Adult day care is eligible.
- You are under the care of a doctor and receiving services under a plan of care created and reviewed by a doctor.
- A doctor certifies you need intermittent skilled nursing care, physical therapy, occupational therapy, or speech therapy.
